Objective

Identifying patients with Post Covid Condition (PCC) in routine practice and in research is usually based on subjective symptoms reported by patient. The use of objective data as a complement to subjective assessment should help to better describe PCC patients. The aim of this study is to characterise PCC using objective measures at 1 year after hospitalization for acute SARS-CoV-2 infection.

Material / Patients and Methods

A prospective, observational, multicentre study was conducted. Patients with SARS-CoV-2 infection, hospitalized at the acute phase and referral to rehabilitation services due to persisting symptoms were included between 20 May and 20 November 2020. Systematic evaluation at 6 and 12 month after infection, with objective measures as muscular testing, handgrip, one minute sit to stand test (1STS), 6-minutes walk test (6MWD), Montreal Cognitive Assessment (MoCA), symbol digit modalities test (SDMT), and subjective measures as Brief Pain Inventory, Hospital Anxiety and Depression scale, Modified fatigue impact scale, fatigue severity scale, dyspnea-12, dyspnea according to the modified MRC and Worl Health Organization Quality of life questionnaire were performed. Analyses included principal components analyses (PCA) and cluster analysis.

Results

Four principal components were extracted and renamed into subjective symptoms, physical capacity, cognitive capacity and quality of social life. Only the use of objective data made it possible to achieve good cluster quality. Three cluster were identified : the recovery cluster, the cognitive cluster and the physical & cognitive cluster. No factors (patient characteristics or acute phase events) were associated with cluster membership.

Discussion - Conclusion

The use of objective measures makes it possible to characterise patients complaining of subjective symptoms and to determine a trajectory for the evolution of post-covid patients at 1 year. Objective measurements should be preferred as subjective one to characterise patient in research and personalize rehabilitation programs.
